Action Required Re: DVF and POC

 

Dear Janet Metcalfe Families,

Please see important messages regarding Data Verification Forms (DVF) and Medical Information – Plan Of Care (POC) below.

  1. Data Verification Form (DVF)

A copy of the Data Verification Form (DVF) was provided to all students today. This form verifies your child’s personal information, as well as your contact and emergency contact preferences. Please complete and return this form to school as soon as possible.

  1. Medical Information – Plan Of Care (POC)

We want to ensure that every precaution has been taken for your child. If your child has medical concerns, you will need to provide details of those concerns, including medications which your child will carry. Please call our school office if you did not receive Plan Of Care (POC) paperwork or require more information.
